Could it simply be an addon overlaying the unit bar? Its tables are framestackable meaning it's addon interactable in a nutshell. AIO can push such addons from a server to client cache and the AIO addon itself can be warden injected to cache on login. I assume it'd be possible to override and always set the health bars to visible but size 1x1 px (or simply make the frame invisible/full opacity) when they are "disabled", and normal size and display when enabled

I'm not sure, I probably have to download that private server and play a little in so figure it out.
They sent this i originally (few mins ago) thought this windows were made with the Eluna AIO mod / addon
