I use Ubiquiti for my router and its setup like this, and it works perfectly, so should be similar to give you an idea.

on the drop down for "destination IP" it shows a lot of my home devices - i dont think i can enter the ipv4 there

I can tell you that IF you have a router from your ISP (cable modem, fiber box) your not gonna get it to work because you will be double natted
