Seems random, some NPC's do and others don't an example is Kraxx 23713 - after speaking with him, he stops pathing and stands still.

They stand still for a certain amount of time but if you don't have creatures set as active in worldserver.conf they won't finish that timer while no players are near

I can confirm this issue right now. With default settings.
I went to deal with the kid and both the npc he meantioned and the 2 blood elves in stormwind are still stopped
