I spent all day figuring out how to do simple model swaps and gearing using the various programs and guides located on mmowned.com. I have succeeded in making a Beautiful Druid Pack.
Here is what is included:
Travel Form
Original: Tiger -----> New: Frostsabre
This is the more detailed cat model, the same as the NE mounts but on a small scale. It has all appropriate animations. Cheetah mobs in places like the barrens may also be affected.
Aquatic Form
Original: Sealion -----> New: Lionseal
This is the awesome model of Ragemane, a blubbery elite. It is way more detailed; you cannot, however, dolphin-scoot the surface of water like you can with the old one.
Tree of Life Form
Original: Ent -----> New: NightElf
This is the night elf model of Arch Druid Malfurion Stormrage. This is way better than a flailing-armed treat. It has a spectral transparency, wears Druid Tier 2 (Tier 1 Helm), and has NE animations.
I owe a great amount of gratitude to reflex0rzor and his amazing GUIDE for model swapping!
I also want to thank Balky for their MODEL SWAP work which, when studied, helped me understand gear swapping.
The original model pictures above are courtesy of wowhead.com.
Download the Beautiful Druid Pack HERE
Use WinRar to extract the patch-d.mpq file and put it in the World of Warcraft/Data folder. Run WoW like normal. You can delete the file to remove the changes.

Edit: Aquatic Form color changed to better reflect current druid aquatic form.
Edit: Tree form transparency lessened to allow character colors to show better.

By the way, where did you find the specific model for Malfurion Stormrage? cos as far as I know he doesn't have a specific model like Jaina or Varian

Malfurion Stormrage appears in-game at two locations (on through a quest) and via spiritual communication since he is still in the Emerald Dream/Nightmare. He has a normal night elf model, much like until recently Jaina was a normal human and Sylvanas a normal night elf then blood elf. I simply duplicated his features and gear by using model viewer to load NPC outfit of him and copied the numbers given into field 3 of DBC. This only works for the same character model though (ie: NE to NE; if i copied his code to a Tauren model it would look way different). Hope this helps.

The thing is the Keeper model does not have very many animations; the only thing you can do with it is run or auto-attack. The actions for sit, dance, jump, etc have no animations associated. Disclaimer applies that any simple cat with cheetah skin (very few) will probably be affected. Don't try to use it with the file I posted above since they affect the same target model.
Unzip with WinRar and extract the .mpq file into the World of Warcraft\Data folder.
